NEUROLOGICAL ASSESSMENT
Remember the old cartoons when someone gets
an idea in their brain and a light bulb would go
off above their head? Think of that, but picture a
LAMP above the patients head
L - LOC (level of consciousness)person,place,time
A - Assess fontanelle (infants under 1 years old
- flat? depressed? bulging?)
M - Movement (hand grasp/pull AND -
pedal dorsiflexion/extension)
P - PERL
SKIN ASSESSMENT
T - Temperature of skin
I - Integrity of skin
M - moisture of skin
E - Edema of skin
Color - Color of skin
* Remember you only have to choose (2)
vulnerable skin areas to assess. They are:
1) heels
2) sacral/coccyx
3) occiput
4) trochanter
5) skin folds
6) peri anal
7) designated area

Please note that you check off the area of care on the LEFT side when you do it, and then check off the area of
care to the RIGHT side when you chart it. That way you won't forget anything!
